Salary: £33,143 – £36,363 per year (pro rata)

Your Role Will Include:
As a Social Care Assessor, you will:
- Receive referrals and carry out holistic social care assessments
- Develop, review, and manage personalised care and support plans
- Promote independence, choice, and control for service users
- Conduct risk assessments, reviews, and self-assessments where appropriate
- Arrange care services, equipment, and minor adaptations
- Work collaboratively with multi-disciplinary teams and community partners
- Maintain accurate records and reports using council systems
- Carry out daily home visits and occasional out-of-hours work
What We’re Looking For:
- Strong communication skills with fluent spoken and written English
- Experience or understanding of adult social care and community support
- Knowledge of social care legislation (MCA, DoLS, safeguarding, eligibility)
- Ability to manage competing priorities in a fast-paced environment
- Confidence in assessment, decision-making, and care planning
- Commitment to equality, diversity, and person-centred practice
- Relevant qualification or equivalent professional experience
- Willingness to travel daily for home visits
