Position: Social Care Assessor

Salary: £33,143 – £36,363 per year (pro rata)

Job Type: Full-time, Fixed-term contract 

Location: Luton (Hybrid)

Your Role Will Include:

As a Social Care Assessor, you will:

• Receive referrals and carry out holistic social care assessments
• Develop, review, and manage personalised care and support plans
• Promote independence, choice, and control for service users
• Conduct risk assessments, reviews, and self-assessments where appropriate
• Arrange care services, equipment, and minor adaptations
• Work collaboratively with multi-disciplinary teams and community partners
• Maintain accurate records and reports using council systems
• Carry out daily home visits and occasional out-of-hours work

What We’re Looking For:

• Strong communication skills with fluent spoken and written English
• Experience or understanding of adult social care and community support
• Knowledge of social care legislation (MCA, DoLS, safeguarding, eligibility)
• Ability to manage competing priorities in a fast-paced environment
• Confidence in assessment, decision-making, and care planning
• Commitment to equality, diversity, and person-centred practice
• Relevant qualification or equivalent professional experience
• Willingness to travel daily for home visits
