What We Offer:
- A Band 3 Health Care Support Worker role within the Community Heart Failure Team
- ‎Part-time position with flexible, negotiable hours (up to 30 hours per week)

Your Role Will Include:
- Work closely with specialist nurses to deliver holistic heart failure care
- ‎Carry out phlebotomy, patient observations, and delegated clinical tasks
- ‎Support clinics, home visits, and community-based care
- ‎Contact patients, arrange appointments, and organise visits
- ‎Manage your own workload while supporting wider team priorities
What We’re Looking For:
- Experience in phlebotomy and patient observations
- ‎Confidence working both independently and as part of a team
- ‎Ability to work across varied community and clinical environments
- ‎Strong communication, organisation, and patient-focused approach
- ‎Commitment to compassionate, person-centred care
