Position: Health Care Support Worker 

Salary: £24,937 – £26,598 a year

Job Type: Full-Time, Permanent 

Work location: Huddersfield, UK

Your Role Will Include:

  • Work closely with specialist nurses to deliver holistic heart failure care
  • ‎Carry out phlebotomy, patient observations, and delegated clinical tasks
  • ‎Support clinics, home visits, and community-based care
  • ‎Contact patients, arrange appointments, and organise visits
  • ‎Manage your own workload while supporting wider team priorities

What We’re Looking For:

  • Experience in phlebotomy and patient observations
  • ‎Confidence working both independently and as part of a team
  • ‎Ability to work across varied community and clinical environments
  • ‎Strong communication, organisation, and patient-focused approach
  • ‎Commitment to compassionate, person-centred care
